PLEASE NOTE: Parents may assist in completing this form but please be mindful that this application is to be completed by or on behalf of the youth interested in attending GEM Culture, Inc.'s 2021 Spring Semester.

GEM Culture, Inc. is offering weekly sessions to the youth and young adults, ages 13-18, to assist and teach them ways to take care of themselves and to remain balanced while we experience the ever-changing environment of life itself. 

All sessions will be offered online; however, a small number of events may take place in person or at times outside of our normal session dates such as our Crowning Ceremony or community service opportunities. 

Upon completing the application form below, participants will be notified whether or not they have been accepted to move onto an interview. First come, first serve. Once the interview portion has been successfully passed and the youth is accepted into the Spring Semester, the youth will be contacted and their parent must complete and sign the parental/registration forms as well as pay the $75 annual registration fee. There are only 25 spots and scholarships are available on a limited basis. All participants must have an electronic device to attend sessions weekly.

SESSION DATES: 
Sessions will take place bi-weekly on Fridays starting Friday, February 26, 2021 til Friday, June 11th, 2021

SESSION TIME: 
5:00pm - 7:00 pm EST

Please be sure to check that your answers are correct before submitting. Please note that this application will not save and must be completed in one sitting. Application opens on Wednesday, December 9th, 2020 and closes on Friday, January 15, 2020 at 12 Noon. Applicants will be notified via phone and email of their acceptance into GEM Culture's 2021 Spring Semester by Thursday, February 11, 2021.
